Consumer Survey

Are you an LCCMHA consumer? If so, we need your help with a survey!

  • We are looking at whether electronic communication can be safely used between you and your service provider (Case Manager, Therapist, Psychiatrist, etc.).
  • Our current policy prohibits us from communicating with you via email or text messaging. Knowing if you are interested in communicating in these ways, and if you have concerns about doing so, will help us as we update our policy.
  • The survey is anonymous--we don't ask any questions about who you are and we won't know anything about you, other than your answers to the questions. Thank you!
